ABSTRACT

Objective To investigate the effects of propofol and sevoflurane on neurological recovery of traumatic brain injury (TBI) patients in the early postoperative stage. Methods We retrospectively analyzed the clinical data of TBI patients who underwent craniotomy or decompressive craniectomy. Generalized additive mixed model (GAMM) was used to analyze effects of propofol and sevoflurane on Glasgow Coma Scale (GCS) on postoperative days 1, 3, and 7. Multivariate regression analysis was used to analyze effects of the two anesthetics on Glasgow Outcome Scale (GOS) at discharge. Results A total of 340 TBI patients were enrolled in this study. There were 110 TBI patients who underwent craniotomy including 75 in the propofol group and 35 in the sevoflurane group, and 134 patients who underwent decompressive craniectomy including 63 in the propofol group and 71 in the sevoflurane group. It showed no significant difference in GCS at admission between the propofol and the sevoflurane groups among craniotomy patients (β = 0.75, 95%CI: -0.55 to 2.05, P = 0.260). However, elevation in GCS from baseline was 1.73 points (95%CI: -2.81 to -0.66, P = 0.002) less in the sevoflurane group than that in the propofol group on postoperative day 1, 2.03 points (95%CI: -3.14 to -0.91, P < 0.001) less on day 3, and 1.31 points (95%CI: -2.43 to -0.19, P = 0.022) less on day 7. The risk of unfavorable GOS (GOS 1, 2, and 3) at discharge was higher in the sevoflurane group (OR = 4.93, 95%CI: 1.05 to 23.03, P = 0.043). No significant difference was observed among two-group decompressive craniectomy patients in GCS and GOS.Conclusions Compared to propofol, sevoflurane was associated with worse neurological recovery during the hospital stay in TBI patients undergoing craniotomy. This difference was not detected in TBI patients undergoing decompressive craniectomy.

ABSTRACT

@#Abstract:Alcoholic liver disease (ALD) is one of the most common liver diseases in the world. Long-term alcoholism causes a series of pathological changes in the liver, which eventually leads to the occurrence of liver diseases with an increasing incidence. At present, significant progress has been made in the pathogenesis and pathological development of alcoholic liver disease, but the relevant mechanism of ALD has not been thoroughly studied. It is necessary to improve the existing animal model or establish a new, more comprehensive animal ALD model to simulate human ALD. Experimental animal models of ALD, especially rodents, are often used to simulate human ALD, and the ideal rodent ALD model can effectively simulate all aspects of alcohol in human liver. But so far, the commonly used animal models all have certain defects, and there is no complete animal model that can simulate human ALD. This paper reviewed the pathogenesis of ALD, related methods and influencing factors of ALD model, and provided a theoretical basis for relevant researchers to establish the ALD rodent model.

ABSTRACT

Traumatic brain injury (TBI), a growing public health problem, is a leading cause of death and disability worldwide, although its prevention measures and clinical cares are substantially improved. Increasing evidence shows that TBI may increase the risk of mood disorders and neurodegenerative diseases, including Alzheimer's disease (AD). However, the complex relationship between TBI and AD remains elusive. Metabolic dysfunction has been the common pathology in both TBI and AD. On the one hand, TBI perturbs the glucose metabolism of the brain, and causes energy crisis and subsequent hyperglycolysis. On the other hand, glucose deprivation promotes amyloidogenesis via β-site APP cleaving enzyme-1 dependent mechanism, and triggers tau pathology and synaptic function. Recent findings suggest that TBI might facilitate Alzheimer's pathogenesis by altering metabolism, which provides clues to metabolic link between TBI and AD. In this review, we will explore how TBI-induced metabolic changes contribute to the development of AD.

ABSTRACT

Objective:To explore the mechanism of rancidity during storage by researching the changes of water content, relative permeability of cell membrane and rancidity levels of Armeniacae Semen Amarum in deterioration process. Method:Armeniacae Semen Amarum samples under different storage conditions were evaluated and classified by sensory assessors, and samples with different levels of rancidity were obtained. Water content was measured by toluene method, and water activity was obtained by water activity meter. Malondialdehyde (MDA) and relative conductivity were measured using thiobarbituric acid colorimetry and conductivity meter, respectively. The content of fatty oil was obtained by Soxhlet extraction. The acid value and peroxide value were measured in accordance with the general rules 0713 and 2303 of the 2020 edition of Chinese Pharmacopoeia (part Ⅳ), respectively. Based on the above experimental data, chemometric methods (cluster analysis, principal component analysis) were selected to establish classification and discriminant models of Armeniacae Semen Amarum with different rancidity levels, in order to verify the accuracy of the classification results. Result:According to the results of sensory evaluation, Armeniacae Semen Amarum samples were divided into three classes, including no rancidity, slight rancidity and rancidity. Compared with the no rancid samples, the color of surface and cotyledon were deepened in rancid samples, and the oil was appeared on surface with rancid taste. The values of water content, water activity, MDA content and relative conductivity were all significantly increased in deterioration process (P<0.01). The content of fatty oil was significantly decreased with the occurrence of rancidness (P<0.01), while the acid value and peroxide value increased significantly (P<0.01). The results of cluster analysis and principal component analysis showed that the rancid samples could be distinguished from the no rancid and slightly rancid samples. Conclusion:The storage conditions under high temperature and high humidity can accelerate the rancidness of Armeniacae Semen Amarum, which is accompanied by the increase of internal water content, the increase of cell membrane permeability and the occurrence of fatty acid rancidity. It is suggested that Armeniacae Semen Amarum should be stored in low temperature, dry environment, as well as short storage time.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate quinalizarin-induced apoptosis in gastric cancer cells in vitro and explore the molecular mechanisms.</p><p><b>METHODS</b>MTT assay was used to determine the cytotoxic effects of quinalizarin on human gastric cancer AGS, MKN-28 and MKN-45 cells. Annexin V-FITC/PI staining and flow cytometry were used to assess quinalizarin-induced apoptosis in AGS cells and its effect on intracellular ROS levels; the expression levels of apoptotic proteins in the cells were determined with Western blotting.</p><p><b>RESULTS</b>Quinalizarin dose-dependently reduced the cell viabilities of the 3 gastric cancer cells (P<0.05). The ICvalues of quinalizarin in AGS, MKN-28 and MKN-45 cells were 7.07 µmol/L, 22.55 µmol/L and 14.18 µmol/L, respectively. Quinalizarin time-dependently induced apoptosis of AGS cells and potentiated the generation of intracellular reactive oxygen species (ROS) levels. Pretreatment with NAC, a scavenger of ROS, inhibited quinalizarin-induced apoptosis (P<0.001). Western blotting results showed that quinalizarin also up-regulated the expression levels of the apoptotic proteins including p-p38, p-JNK, Bad, cleaved caspase-3, and cleaved PARP-1 (P<0.05), and down-regulated the expression of the anti-apoptotic proteins p-Akt, p-ERK, and Bcl-2 (P<0.05).</p><p><b>CONCLUSION</b>Quinalizarin inhibits the proliferation and induces apoptosis in gastric cancer cells in vitro through regulating intracellular ROS levels via the MAPK and Akt signaling pathways.</p>

ABSTRACT

Objective To retrospectively analyze the perioperative complications of posterior lumbar interbody fusion (PLIF) for recurrent lumbar disc herniation and identify potential risk factors that correlate with those complications. Methods All of 71 patients with recurrent lumbar disc herniation were treated surgically with PLIF,discharged from our department between January 2008 and December 2014.Demographic and operation data were collected and perioperative complications were recorded. We analyzed whether the clinical factors (age,gender,BMI,co-morbidity,smoking,time of recurrence, blood loss, operation segment, operation time) were in correlation with perioperative complication by univariate analysis. Then we integrated the statistically significant indicators into Logistic regression equation to determine the related risk factors for complication. Results The study group consisted of 71 cases, including 42 males and 29 females. The age was 19 to 64 years old with an average of 50.6 years old,and the average BMI was 23.6 kg/m2.26 cases had perioperative complications, while there were two or more complications in 5 patients, and no mortalities.Neurologic deterioration or neuropathic pain (10 cases,14.1%) and dural tears (6 cases,8.5%) were the most common intraoperaitive complications. The other complications included nerve root or cauda equine injury, deep or superficial wound infection, urinary tract infection, respiratory system complication,cardio-vascular complication and delirium.Univariate analysis suggested that age,gender,co-morbidity,smoking,time of recurrence, operation segment, operation time were not associated with perioperative complication. The multivariate Logistic regression analysis showed BMI and blood loss were closely related to perioperative complication. Conclusion Complications of posterior spinal fusion surgery for recurrent lumbar disc herniation are affected by many factors. The most common complications are transient neurologic deterioration or neuropathic pain and dural tears.BMI and blood loss are independent risk factors.

ABSTRACT

Coptisine hydrochloride, as a natural protoberberine alkaloid quaternary ammonium salt, can be found in many species of Ranunculaceae and Papaveraceae plants. Despite no in-depth studies on coptisine hydrochloride, some literatures have reported that coptisine hydrochloride has such pharmacological activities as inhibition of monoamine oxidase of type A, selective inhibition and double inhibition against vascular smooth muscle cell proliferation, inhibition of differentiation and function of osteoclasts, selective regulation of multidrug-resistant and drug-resistant proteins in vascular smooth muscle cells, anti-fungus, protection of gastric-mucous membrane, cytotoxicity, and myocardial protection. Given to the fact of the lack of systematic review and summary of studies on coptisine hydrochloride, we summarize and analyze the study literatures on the pharmacological activity of coptisine hydrochloride published in recent years, so as to provide information for studies on new drugs of coptisine hydrochloride on the basis of the pharmacological activity.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the quality of life and influencing factors on patients with multiple myeloma (MM).</p><p><b>METHODS</b>227 MM cases were selected at 5 hospitals in Xi'an from August, 2010 to March, 2013. QLQ-C30 was used to evaluate the quality of life of MM patients, and their norms were as control. Factors which influencing the quality of life were investigated and analyzed with SPSS 17.0 software.</p><p><b>RESULTS</b>The total score of quality of life in MM patients was 49.0±21.7 which was lower than the norms (60.7±23.4). The scores on fatigue, nausea, vomiting, pain, short of breath, disturbance on sleeping, losing appetite, constipation, other symptoms and financial difficulty were significantly higher than data of the norms (P < 0.05). Factors as being elderly (especially those older than 70), under higher proportion of medical costs on their own expense or financial difficulty etc., had major influences on the quality of life (P < 0.05) of MM patients who in particular having worse quality of life when in worsening clinical ISS stage (P < 0.05). Low level of hemoglobin, high level of serum calcium and globulin all significantly reduced the quality of life of the MM patients (P < 0.05).</p><p><b>CONCLUSION</b>The quality of life of MM patients was significantly lower than the normal people or patients with other tumors. Fatigue, pain, and financial difficulty were main influencing factors on the quality of life of MM patients. The assessment on the effects of treatment should relate to the improvement of hemoglobin, serum calcium and globulin, which could all improve the quality of life of MM patients.</p>

ABSTRACT

Objective To observe the operative effect of selective decompressive laminectomy and long segment pedical screw internal fixation for degenerative scohosis beyond 3 segments. Methods During January 2003 to July 2009, 52 cases with degenerative scoliosis beyond 3 segments were treated by posterior selective decompressive laminectomy, intervertebral implanted bone fusion and long segment pedical screw internal fixation, vetebral plate fusion seperately, and all the cases were followed up. Results All the patients were followed up for 12-60 months after operation. The status of back leg pain relief were evaluated through Oswestry score system. The Oswestry score of 1 week,3 months and 12 months after operation were (25.91 ± 9.05), (26.75 ± 8.37), (26.90 ± 8.45) scores respectively and there was statistical significance compared to the preoperative [(46.35± 8.12) scorns] (P <0.05). The average coronal Cobb angle was corrected from 15.23°± 3.35° to 10.02°± 3.26° 1 week after operation (P < 0.05). All the internal implant didn't appear loosing or fracture confirmed by X-ray at 3,6,12 months after operation. The intervertebral implanted bone fusion and vetebral plate fusion all got well in 18-24 months after operation. Conclusion Selective decompressive laminectomy and long segment pedical screw internal fixation for the degenerative scoliosis can get better operative effect.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the efficacy and safety of a new method which determines the exact distal fusion level in the treatment of adolescent idiopathic scoliosis (AIS) with posterior pedicle screw fixation and to assess its clinical outcome.</p><p><b>METHODS</b>This prospective clinical study analyzed 31 AIS patients who met the inclusion criteria enrolled from July 2005 to September 2008. Based on the principle of our new criteria for selection of distal fusion level of AIS, all patients had posterior spinal fusion and instrumentation with pedicle screws. Cobb angle of the curve, tilt angle of the LIV, intervertebral angle and trunk shift were measured and analyzed.</p><p><b>RESULTS</b>Preoperative (42 + or - 17) degrees of thoracic curve was corrected to (12 + or - 7) degrees , with a curve correction of 70.6%. Preoperative (44 + or - 7) degrees of lumbar curve was corrected to (9 + or - 4) degrees , with a curve correction of 80.2%. The trunk shift were significantly improved from (13 + or - 8) mm to (9 + or - 7) mm before and after surgery respectively (P < 0.05). The tilt angles of the LIV before and after surgery were (20.8 + or - 5.7) degrees and (1.5 + or - 3.1) degrees respectively. The thoracic Cobb angle was (14 + or - 8) degrees and the lumbar Cobb angle was (9 + or - 5) degrees at latest follow up. The changes were of significance in the tilt angle of the LIV after surgery compared with that before surgery (P = 0.000). This angle averaged (0.8 + or - 3.7) degrees at final follow up, but the change was not significant compared with that after surgery (P > 0.05).</p><p><b>CONCLUSION</b>This is an effective method with the advantage of shortening the fusion level, reserving the distal motion segments and easing segmental degeneration adjacent to the fusion area.</p>

ABSTRACT

<p><b>BACKGROUND</b>Patients with congenital scoliosis often also have intraspinal abnormalities and other organ defects, and few studies of the effects of congenital scoliosis on cardiac function and structure have been published.</p><p><b>METHODS</b>A total of 215 adolescent patients with congenital scoliosis (average age, 13.58 years) underwent preoperative echocardiography and were then assigned to subgroups according to apex vertebral rotation, side of convexity, curvature severity in the coronal and sagittal planes, type of deformity, and sex. Differences between the subgroups were compared by independent-samples t test or a one-factor analysis of variance.</p><p><b>RESULTS</b>We observed statistically significant differences between patients with right-sided scoliosis curvature and those with left-sided scoliosis curvature, respectively, in left ventricular inner diameter at end-diastole ((39.39 +/- 4.66) mm vs (41.74 +/- 4.90) mm), left ventricular inner diameter at end-systole ((24.80 +/- 3.45) mm vs (25.92 +/- 3.07) mm), interventricular septum thickness at end-diastole ((5.66 +/- 0.98) mm vs (5.98 +/- 1.03) mm), and posterior wall of left ventricle at end-diastole ((5.61 +/- 0.98) mm vs (6.06 +/- 1.20) mm). When the patients were evaluated by coronal plane Cobb angle, significant differences were found between those with Cobb angle of 40 degrees - 80 degrees and of > 80 degrees in left ventricular inner diameter at end-diastole ((40.97 +/- 5.06) mm vs (38.98 +/- 4.45) mm) and left ventricular inner diameter at end-systole ((25.53 +/- 3.39) mm vs (24.36 +/- 3.14) mm), respectively. When the patients were evaluated by sagittal plane Cobb angle (< 20 degrees, group 1; 20 degrees - 40 degrees, group 2; > 40 degrees, group 3), significant differences were found in right ventricular diameter between those with Cobb angle of < 20 degrees and of 20 degrees - 40 degrees ((18.27 +/- 3.66) mm vs (16.54 +/- 3.57) mm) and in diameter of aortic root between those with Cobb angle of 20 degrees - 40 degrees and of > 40 degrees ((23.83 +/- 3.39) mm vs (24.90 +/- 3.30) mm), respectively. No significant differences were found in ejection fraction and fractional shortening between patients according to apex vertebral rotation, side of convexity, coronal plane and sagittal plane Cobb angles, type of deformity, or sex.</p><p><b>CONCLUSIONS</b>Congenital scoliosis influences cardiac structure, but not function.</p>
